Health assessments

The Public Health School Nursing Service works together with children, young people and their parents/carers to provide a comprehensive efficient and accessible service.  We offer health assessment at transition points throughout a child’s life.

Primary health assessments

We offer health assessments for children before a child starts primary school and in Year 6.  These help parents, carers or children identify anything that may impact on a child’s ability to achieve their potential. 

The assessment can help to support a child’s smooth transition into primary or secondary school.  We can provide advice, guidance and support for a range of issues such as behaviour, healthy lifestyles, growing up, puberty, toileting and emotional wellbeing.

Secondary health assessments

We offer confidential health assessments for young people in Year 9 (aged 13-14yrs) and Year 12 (aged 16-17yrs) to identify any advice, support or help that might be needed to prepare them for adulthood.

This can include a range of health issues such as sexual health, healthy lifestyle, emotional health and wellbeing, bullying and any general health concerns. This support can give young people strategies to build resilience and to manage difficult situations and stress.
